Welcome to Dr. V’s Queens Botox, Juvederm, Morpheus, Hair Transplant & IV Drip Med-Spa (“we,” “our,” or “us”). By accessing or booking through mymdspa.com, you agree to the following terms and conditions.
1. Medical Disclaimer
All treatments and services are performed by licensed medical professionals. Results vary per individual and are not guaranteed.
Our IV drips, injectables, and other services are for elective wellness or aesthetic purposes only and do not substitute for emergency medical care. If you have a medical emergency, call 911.
2. Appointments & Payments
Appointments can be booked directly on our website or through Practice Fusion, our secure medical scheduling and record platform.
A valid credit card or deposit may be required to confirm your booking.
Cancellations made less than 24 hours before the appointment, or missed appointments, may incur a cancellation fee.
Services must be paid in full at the time rendered unless otherwise arranged.
3. Use of Practice Fusion
We use Practice Fusion, a HIPAA-compliant electronic health record (EHR) and scheduling platform, to manage patient bookings, medical records, and treatment documentation.
When you schedule or complete an appointment, your information may be securely transmitted to and stored within Practice Fusion’s system in accordance with their Privacy Policy and Terms of Use.
Practice Fusion’s platform ensures compliance with federal and state privacy regulations, including HIPAA.
You agree that by booking an appointment, you consent to your personal and medical information being handled in accordance with Practice Fusion’s and Dr. V’s Med-Spa’s privacy and data protection policies.
4. Refunds
All services rendered are non-refundable. If a treatment cannot be completed for medical or clinical reasons, we will review the case according to our Refund Policy and applicable law.
5. Informed Consent & Medical Evaluation
All medical services, including IV drips, injectables, and skin procedures, require a medical evaluation and informed consent prior to treatment.
You must provide complete and accurate medical information before receiving care.
Our providers reserve the right to decline or modify treatment if contraindications exist.
6. Regulatory Compliance (New York & New Jersey)
Our medical spa operates in compliance with New York State and New Jersey medical and business regulations, including the Corporate Practice of Medicine (CPOM) laws.
Only licensed physicians may own or control medical operations and direct patient care decisions.
All services are performed under proper medical supervision and within the scope of each provider’s license.
Our facilities and staff adhere to all applicable Department of Health and local licensing standards.
7. Privacy & Data Security
We protect your Personal Health Information (PHI) in accordance with HIPAA and New York State privacy laws.
Medical data is maintained within Practice Fusion’s secure environment.
Non-medical contact information collected on mymdspa.com (e.g., email or phone for inquiries) is stored separately and used only for appointment coordination or service communication.
We do not share or sell your personal information to third parties.
8. Intellectual Property
All website materials — including text, design, graphics, photos, and branding — are owned by Dr. V’s Med-Spa.
You may not copy, modify, or redistribute any materials without our written consent.
9. Limitation of Liability
To the fullest extent permitted by law, we disclaim any warranties regarding outcomes, results, or reactions to treatments.
Our liability is limited to the amount paid for the specific service in question.
10. Governing Law
These Terms are governed by the laws of the State of New York, and where applicable, the laws of New Jersey.
Any disputes shall be resolved in the courts of New York County, NY, unless otherwise required by law.
11. Updates to These Terms
We may modify these Terms from time to time. Updated versions will be posted on our website with the effective date. Continued use of our site or services after updates constitutes acceptance.
